When you come in for your initial appointment at Brooklyn Endovascular, please remember that you will need to fill out new patient forms, including a medical history questionnaire. Please arrive 30 minutes early and bring your photo ID and insurance card. If your insurance plan requires a copayment, you will need to pay it when you check-in.
