Please come to your first appointment at Fishkill Endovascular 30 minutes early with your photo ID and insurance card. If your insurance has a copay, it will be collected at the time of check-in. When you arrive at our affiliated Fishkill center, you will be expected to complete new patient forms, including a medical history questionnaire.
